Hormonal Changes and Libido: A Life-Stage Overview

Libido is regulated by a combination of estrogen, testosterone, and psychological factors, and it shifts in fairly predictable ways across a woman's life, even though individual variation is significant.

During perimenopause, which can begin up to a decade before menopause itself, declining and fluctuating estrogen levels reduce vaginal lubrication and tissue sensitivity, often before other menopause symptoms become apparent. This is a physiological change, and lubricant use plus, where appropriate, topical or systemic hormone therapy are standard clinical responses.

Testosterone, though present at lower levels in women than men, plays a measurable role in spontaneous desire. Age-related decline can contribute to reduced libido independent of estrogen changes, which is one reason some women are prescribed testosterone therapy under medical supervision.

Medication effects deserve specific mention: SSRIs and combined hormonal contraceptives are both well-documented causes of reduced libido in a meaningful subset of users. A change in desire that coincides with starting a new medication is worth discussing with a prescriber rather than assumed to be unrelated.

The clinical takeaway is straightforward: libido changes across life stages are largely physiological, generally have identifiable causes, and are frequently manageable once the underlying driver is identified.
